Descriptions of Duties
- To deliver patient-centred pharmaceutical care, including but not limited to the management of minor ailments, medication management, outreach and teleconsultations;
- To oversee the operation of the community pharmacy to ensure compliance to the legal requirements and code of practice;
- To process, verify, dispense and document prescriptions in accordance to legal requirements;
- To contribute to holistic patient care and services at the Kwai Tsing Community Health Management Hub and Wofoo Wellness Hub as a team member of transdisciplinary teams;
- To engage patients and caregivers via social media, educational activities and community support programmes;
- To innovate and plan primary care pharmacy services to address community-specific health needs;
- To work with the Department of Pharmacology and Pharmacy, The University of Hong Kong on collaborations on service, training and research.
Requirements
- A Bachelor Degree in Pharmacy or equivalent awarded by institutions recognised by the Pharmacy & Poisons Board of Hong Kong
- Preferably registered as a Hong Kong pharmacist with the Certificate of Registration and a valid Practising Certificate issued by the Pharmacy and Poisons Board of Hong Kong
- Enthusiastic, self-motivated and highly organised with good team works
- Passionate in primary care healthcare initiatives
- Excellent command of both spoken and written English and Chinese.

Terms and Conditions
- Appointed visiting pharmacist have to comply with the internal operating procedures, guidelines and policies of Health In Action
- The commitment as a visiting pharmacist is made on honorary basis, however volunteer allowances may be provided in specific duties and projects
- Work schedule and number of duty hours are subject to mutual agreement with flexibility
- Appointee shall notify Health in Action for any change or termination of the commitment with prior notice of at least two weeks
